“Enhancing the Quality of Life for our Elderly”
Universal’s Long-Term Care Community Diversion Program
Universal Health Care has a Long-Term Care Community Diversion Program. The program is for
members who qualify for Medicaid nursing home placement. It keeps people from being placed
in nursing homes when not needed. It has services to help members stay at home or in an
Assisted Living Facility.
Universal’s Diversion program has many services. These services help members take care of
themselves. Some services can help members become more independent. Other services can
make members’ lives easier.
Universal’s program also has Case Managers. They gather information from the members’
doctors and families. This information helps members with their needs.
