Hypertension (high-blood pressure) is diagnosed in one out of four adults. Hypertension
can be controlled. Universal Health Care has a free program for our members who
have been diagnosed with Hypertension. Members who are enrolled will receive periodic
calls from our nursing staff to reinforce the proper treatment plan and educate
our members on the early signs and symptoms of Hypertension. The nurse will discuss:

• Signs and symptoms
• Medications and medication adherence
• Following a low-sodium (salt) diet
• Doctor appointments
• Prevention of hospital admissions
• Monitoring and recording blood pressures

If you are currently a Universal Health Care member who has Hypertension and would
like to sign up for the Disease Management Program, please fill out the information
