VitalifeSM Program Information
Universal Health Care Group is pleased to announce that in addition to offering VitalifeSM
to our Medicare Masterpiece members it is now available to our Private-Fee-For-Service plan
also known as The Any, Any, Any Plan®. Beginning January 1, 2009 all Medicare participants will be
able to enjoy a free membership at one of our select fitness facilities at no additional cost to the
member. Your membership includes unlimited use of the equipment and exercise classes
that do not require an additional fee for participation.
To take advantage of this benefit, simply print and complete the 'VitalifeSM
Registration' form accessed by the link below and have your Primary
Care Physician sign it. Once completed, mail it to the address provided at the bottom
of the form. After you receive your membership card and Welcome Letter with your
start date in the mail, you may take full advantage of the fitness center programs,
classes and receive one free fitness orientation. Please remember to always swipe
your Wellquest ID card when entering your gym; this helps us keep the free gym membership
available to you by confirming that our members are using the VitalifeSM
benefit. Some locations may vary; please ask at your primary facility. You are not
required to join one of our fitness facilities if you do not wish to.
The facility benefit is not limited to the primary facility you indicated on your
VitalifeSM Registration form. VitalifeSM on the Go allows
you to transfer your membership to another location while you are away from your
home state and to transfer your membership back once you return. Directions to access
your facility membership and/or how to travel with your membership are included
in the Facility listing.
Universal Healthcare Group also offers free membership with an interactive online
Wellness program at no additional cost to the member. This online program offers
a wide assortment of information and tools designed to help you achieve excellent
health! You may choose to participate in one of the 6 week programs to lose weight, improve nutrition,
and quit smoking. Internet access is required to enjoy this benefit and you are
not required to belong to a fitness facility in order to participate in the online
program.
Florida residents also enjoy the opportunity to attend our Wellness seminars and
workshops. The VitalifeSM team coordinates events with professional speakers
on topics related to your health, nutrition, and safety. Our topics currently include
General Nutrition, Fall Prevention, Personal Safety, Exercise, and Meal Planning.
The VitalifeSM team is constantly working to provide new topics and more
information in response to our membership’s requests. Seminars and workshops will
be held in areas with enough membership to support the event; as we grow our membership
base VitalifeSM will offer more presentations in more locations. Look
for your invitation to arrive in the mail when we come to your area!
